There are a number of scenarios in which an error can occur when generating a report. This can probably be fixed with a combination of better error handling to catch whatever the error is for a report failure and better form validation.

Specifically for the validation:

• The create button should be disabled if an entity type is not selected

• The UUID should be validated as a valid UUID
